12 Week Challenge:  June 2nd to August 16th

Teams of 3
The team is to include one person who will be your team captain.  The team captain is responsible to coordinate team weigh-in, collects team entry fees, pick up scorecards, reports weights and points to MNbump Wellness Challenge Leaders.

Entry Fee:  $10.00 per person

Note:  Entry fee is cash only to your Team Captain, the fee is due to Leaders no later than June 14th in order to participate.

Team captain can complete the registration form or contact the Wellness Challenge leaders to register the team.

Participation scorecards are near complete. I will post when they are ready and more details where to pick them up.

– Initial weigh in and registration to be reported no later than June 9th at 8:00 pm
– Mid-weigh in to be reported the week of July 7th, final report date July 14th at 8:00 pm
– Final weigh in to be reported no later than August 16th at 8:00 pm.

Note from Sara Tollakson:
I know that summer is busy and likely many will be gone for a week or more during this time. I don’t want this to discourage you from participating. The weigh-in timeline is below. If you know that you will be gone during these times – your options would be to either weigh prior to the weigh-in week, or to weigh individually and add weight to the team’s weight, or all weigh individually and add weights together (team captain will be responsible for collecting this information to report to the wellness challenge leaders). It’s appreciated if you use the same scale to weigh in each time, but not required. (and the same amount of clothing each time, example – if you wear jeans, sweatshirt, and boots the first time, that is what you wear for mid and final weigh-in). The points activities, for the most part, you will be able to do at your own pace and time – so should not be a conflict with your schedule. There are scheduled events that may interfere, but there are plenty of other opportunities to earn points. Locations of weigh in’s are pending. I will share that information as I get it

Prizes
100% of entry fees will be evenly divided for prizes for teams with the most points earned and highest percent weight lost.

About the Challenge: 
Ortonville Area Health Services (OAHS) is hosting this event through the leadership of Sara Tollakson.  Sara is the Health Coach/RN Care Manager.  She provides individualized care management and Health Coach Services, ensures that patients receive the resources and support they need to stay healthy, improve health, and manage their healthcare.

MNbump bloggers are assisting by way of community connection and communication.
